Nineteen primary care providers were awarded a total of more than $5 million for improving care and reducing state Medicaid costs by $34 million, the Department of Human Services announced at an event at the State Capitol Friday.

The providers are participants in the state’s Patient-Centered Medical Home program, which is a team-based care model led by a primary care provider. Providers were given a per-member, per-month payment in order to better manage patients with chronic conditions, provide care for the top 10% of patients with complex needs, meet patient care and technical metrics, provide round-the-clock telephone access, coordinate care for high-risk patients, and ensure patents receive post-hospital care.

The largest award of $927,642.50 went to UAMS Regional Programs serving Fort Smith, Fayetteville, Springdale, Jonesboro and Texarkana. Arkansas Medicaid Medical Director Dr. William Golden said in the press release that providers were reporting that patients are better controlling high-cost health conditions, and fewer are being readmitted into hospitals.

According to a press release from the Department of Human Services, the program enrolled 659 primary care physicians in 123 practices serving 295,000 Medicaid beneficiaries during this first year.

Thirty-seven providers qualified for the awards because they covered 5,000 Medicaid beneficiaries alone or in concert with another practice. Nineteen met reporting and quality metrics and received awards.
