Donna Marie Mantooth

Donna Marie Mantooth, 67, of Fort Smith, AR, passed away on Sunday, December 28, 2025. She was born on October 11, 1958, in Long Beach, CA to the late Gerald and Helena Furka Lenzie. Donna retired from Legacy Health and Rehabilitation, having worked for over 40 years as a CNA in different nursing homes. She was a Catholic. Donna was an amazing CNA, a hard worker, giving, and a very strong person. In addition to her parents, Donna is preceded in death by a brother, Michael Lenzie.

She is survived by two sons, Chris Mantooth and his partner Sara Johnson, and Adam Mantooth, all of Fort Smith. Two sisters, Patricia De Falcon of Palm Beach Gardens, FL, and Lore Valdez of Florida; and a brother, John Lenzie of Tannersville, PA. She is also survived by her grandchild, Lily Ebel of Fort Smith.

There will be a private family held celebration of life at a later date. Cremation arrangements are under the direction of Edwards Funeral Home of Fort Smith.
